If your venue, organization, municipality, school, or event location requires a Certificate of Insurance (COI) for your rental, you can request one using the form below.
A Certificate of Insurance, commonly referred to as a COI, is a document that provides proof of insurance coverage. Some venues, organizations, municipalities, schools, and other facilities require a COI before allowing a rental or event to take place at their property.
You may need a COI if your venue or organization requires proof of insurance before your event. If you are unsure, check with your venue or event coordinator before submitting your request.
If your venue requires them to be listed as an Additional Insured, you will need to provide the exact legal name and address that should appear on the certificate.
Please make sure all information is entered exactly as provided by the venue or organization. Incorrect information may result in delays or require the certificate to be corrected and reissued.
COI requests should be submitted as early as possible before your event. Requests requiring special wording, limits, or additional insurance requirements may require additional processing time.
Complete the two steps below to submit your Certificate of Insurance request.
